Job summary

Wellstar Health System is seeking a Hospice RN Case Manager in Georgia to provide nursing care for terminally ill patients and support their families. You will assess, plan, implement and evaluate care with the hospice interdisciplinary team, coordinating with physicians, social workers, chaplains and volunteers.

This role requires an RN license (Georgia or Compact), BLS, and at least 2 years of acute care or hospice/home care experience.

Qualifications

• Associates or diploma in nursing, or Bachelor's degree preferred.
• RN license in Georgia (single state) or compact license.
• BLS certification required.
• CHPN certification preferred.
• Minimum 2 years acute care or hospice/home care experience.
• Strong assessment and clinical judgment skills.

Responsibilities

• Provides ongoing patient care per the interdisciplinary care plan.
• Coordinates care with physicians, social workers, chaplains, and volunteers.
• Educates caregivers on disease processes, symptom management, and medications.
• Manages medications and safety as ordered; follows DEA guidelines.
• Documents in the EMR and maintains HIPAA-compliant records.
