Job summary

Wellstar Health System in Powder Springs, GA is seeking a Hospice RN Case Manager who uses the nursing process to coordinate care for terminally ill patients and support their families. Qualifications

• RN with 2+ years in acute care, hospice, or home care.
• Active RN license (single state or compact) and BLS.
• Valid Georgia driver’s license and private vehicle insurance.
• Strong assessment, clinical judgment, and communication skills.

Responsibilities

• Provide ongoing patient care per the interdisciplinary plan.
• Maintain contact with patients/families (calls, visits).
• Educate on disease processes, symptoms, medications, and nutrition.
• Manage medications and ensure proper storage and DEA compliance.
