Job summary

Wellstar Health System seeks a Hospice RN Case Manager to oversee comprehensive patient care for terminally ill individuals and their families. You will assess, plan, implement and evaluate care in collaboration with the primary physician, Hospice Medical Director and interdisciplinary team.

Qualifications

• Possesses emotional maturity and strong interpersonal skills.
• Demonstrates excellent assessment and clinical judgment.
• Knowledge of developmental characteristics across ages (neonate to geriatric).

Responsibilities

• Provide ongoing patient care per the interdisciplinary care plan.
• Maintain regular contact with patients and families (calls, visits).
• Educate on disease processes, symptom management, medications, and nutrition.
• Coordinate care with hospice team and community partners.
