Job summary

Wellstar Health System in Georgia is seeking a Hospice RN Case Manager to lead patient care using the nursing process (assess, plan, implement, evaluate) for terminally ill patients and their families in collaboration with the primary physician and the hospice team.

You will coordinate care with interdisciplinary partners, provide physical and psychosocial support, educate caregivers, and prepare families for death. A personal vehicle is required with mileage reimbursement.

• Proficient with computer-based patient care documentation.

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan.
• Maintains ongoing contact with patients/families (phone calls, visits).
• Educates caregivers on disease processes, symptom management, and medications.
• Assesses symptoms and medication effectiveness, reports to physician.
• Facilitates transfer to appropriate care facility as needed.
• Maintains current documentation in electronic medical record.
