Job summary

Wellstar Health System in Georgia is seeking an experienced Hospice RN Case Manager to coordinate end-of-life care with the interdisciplinary team. You will assess, plan, implement, and evaluate patient care for terminally ill patients and support families throughout the death and bereavement process.

Expect teamwork, strong clinical judgment, and the ability to educate caregivers. A valid RN license, BLS, and reliable vehicle are required for this on-site role with mileage reimbursement.

Qualifications

• Must have active RN license
• BLS certification required
• Experience in hospice/home care preferred
• Excellent communication with patients, families, and team

Responsibilities

• Provides ongoing patient care per the interdisciplinary care plan.
• Maintains contact with patients and families (calls, visits).
• Educates regarding disease processes, symptom management, medications, and nutrition.
• Manages medications and safe storage, educates on DEA guidelines.
• Assesses symptoms and reports to physician; documents disease progression.
• Evaluates level of care needs and facilitates transfers as needed.
• Ensures patient safety and caregiver support; assesses for abuse/neglect and suicide risk.
• Documents plan of care and coordinates with CCPs and care team.
• Participates in team meetings and family conferences.
• Maintains EMR documentation and participates in performance improvement.
