Job summary

Heartland Hospice is seeking a compassionate RN Case Manager to oversee patient care in home and community settings. You will lead comprehensive assessments, develop tailored plans of care, and coordinate with physicians, social workers, and other team members to ensure high-quality end-of-life care.

Ideal candidates hold an active RN license, CPR certification, and have experience in hospice or palliative care.

Qualifications

• Current RN license in the state of employment.
• CPR certification required.
• Hospice or palliative care experience preferred.
• Strong communication with patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care across interdisciplinary teams.
• Educate patients and caregivers on disease and care.
• Communicate condition changes to providers promptly.
• Support care transitions and referrals as needed.
• Document timely clinical records and care plans.
• Maintain IDG relationships and team collaboration.
• Participate in on-call and patient support activities.
