Job summary

Heart of Hospice, part of LHC Group, is seeking an experienced Registered Nurse Case Manager to deliver and coordinate the Plan of Care for hospice patients. You will assess needs, collaborate with the care team, and ensure compassionate, quality end-of-life care in patients' homes.

This Mississippi role requires an active RN license in Mississippi, a valid driver's license with reliable transportation, and typically 1+ year of RN experience in a healthcare setting; CPR certification is

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• MS state requirement: 1+ years RN experience; LPN substitution available.

Responsibilities

• Make initial nursing evaluation to determine eligibility for hospice within 48 hours of referral.
• Identify patient and family needs; re-assess at least every 15 days.
• Initiate and coordinate the plan of care with IDG and attending physician.
• Document goals, interventions and patient/family response to care.
• Supervise patient/family self-care teaching and maintain clinical notes.
• Collaborate with patient/family and care team to deliver daily care.
