Job summary

Lutheran Social Ministries of New Jersey (Journey Hospice) is seeking an RN Case Manager to cover a Downingtown, PA caseload. This full-time role offers competitive pay, benefits and a strong work-life balance within a compassionate hospice team.

The RN will perform initial and ongoing nursing assessments, develop and implement the IDT plan of care, educate patients and families, and coordinate with the interdisciplinary team to meet patient needs and ensure quality care.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• PA RN licensure in good standing.
• CHPN certification preferred when available.
• Experience in hospice/home health or related areas preferred.
• Ability to work within an interdisciplinary team to deliver patient-centered care.

Responsibilities

• Oversee primary hospice caseload and provide skilled nursing care.
• Perform initial and ongoing nursing assessments and plan of care.
• Collaborate with the Interdisciplinary Team to implement and reevaluate plans.
• Educate patients and families about care needs and treatment options.
• Supervise Hospice Aides/Homemakers and coordinate with other staff.
