Otterbein SeniorLife is seeking a Hospice RN Case Manager in Ohio to manage a primary caseload and coordinate comprehensive end-of-life care. The role follows Hospice Medicare Conditions of Participation and Ohio licensure rules, working with families and an interdisciplinary team.

The ideal candidate will have 2–4 years of hospice care and strong nursing leadership, with a compassionate approach to patient comfort and family support.

Qualifications

• Bachelor's degree in nursing required or preferred.
• Valid Ohio RN license.
• 2–4 years direct patient care in hospice and case management preferred.

Responsibilities

• Manage a primary caseload of hospice patients.
• Provide skilled nursing care per physician orders and Hospice Medicare guidelines.
• Collaborate with family/caregivers and interdisciplinary team to develop care plans.
