Job summary
OhioHealth at Home in partnership with Compassus is seeking a Hospice Registered Nurse / RN Case Manager to deliver compassionate home-based hospice care. You will build trusting relationships with patients and families, educate caregivers, and collaborate with the care team to provide comfort and dignity.
Eligible applicants have an RN license, CPR/BCLS certification, a current driver's license, and 2 years of nursing experience (hospice preferred).
Qualifications
- RN license required for practice in the state
- Two years of nursing experience with hospice experience preferred
- Current CPR and BCLS certification required
- Valid driver’s license required
Responsibilities
- Provide compassionate hospice nursing care to patients in home settings
- Educate families and caregivers about care plans and symptom management
- Collaborate with care team to deliver comfort, peace, and dignity to patients
- Support patients and families to enable meaningful time with loved ones
