Job summary

Otterbein SeniorLife in Ohio is seeking a Hospice RN Case Manager to join our mission-focused Hospice team. You will manage a primary caseload and deliver skilled nursing care to patients and families under physician orders, in compliance with hospice guidelines and Medicare conditions of participation.

This full-time role follows a regular schedule (8a–5p, Monday–Friday) with on-call rotation, and offers health benefits, retirement savings, tuition assistance, and paid time off to support a

Qualifications

• Education: Bachelor's degree in nursing; Graduate of approved nursing program.
• Licensure: Valid Ohio Registered Nurse (RN) license.
• Experience: A minimum of 2–4 years’ direct patient care experience in hospice; case management preferred.
• Experience: End-of-life diagnosis experience preferred.

Responsibilities

• Manage a primary caseload for hospice patients.
• Provide skilled nursing care to patients and families as prescribed by the physician and in compliance with hospice guidelines.
