Job summary

Ohio Living is seeking an RN Case Manager for Hospice to plan, coordinate, and deliver high-quality hospice care in line with state and federal standards. The role includes home and facility-based care, ensuring symptom management and strong communication with the care team.

The ideal candidate holds an Ohio RN license with at least one year in hospice, is proficient in Windows, MS Office, and EMR systems, and can educate patients and families. Travel and on-call work are possible.

Qualifications

• Current unencumbered Ohio RN license.
• Minimum one year hospice experience.
• Proficiency with Windows, MS Office, and the internet.
• EMR experience preferred.

Responsibilities

• Care management and service delivery: assess patient needs and develop a care plan.
• Follow nursing standards and hospice protocols.
• Provide symptom management for patient comfort.
• Communicate changes in condition with other providers.
• Assess patient and caregiver needs; provide education.
• Commit to high quality customer service for internal/external customers.
• Oversee implementation of the Plan of Care and evaluate effectiveness.
• Prepare for IDG by reviewing Plan of Care and reconciling medications.
