A qualified candidate will have an RN license in Ohio and experience in hospice or case management. The position offers full-time hours with on-call and weekend rotation, with comprehensive benefits and opportunity to serve in a mission-driven, not-for-profit

Qualifications

• Bachelor’s degree in nursing (preferred).
• Valid Ohio RN license.
• 2–4 years direct patient care experience in hospice/case management preferred.
• End-of-life care experience is preferred.

Responsibilities

• Provide skilled professional nursing care to patients and families as prescribed by the physician and in compliance with Hospice regulations and agency policies.
• Manage a primary caseload with on-call and weekend rotation.
• Collaborate with interdisciplinary team to develop and implement individualized care plans.
