Ohio Living is seeking an RN Case Manager for Hospice to plan and deliver high-quality hospice care in patients’ homes and facilities across Ohio. You will assess needs, develop care plans, manage medications, educate families, and collaborate with the health team to ensure compassionate, compliant care.

Qualifications

• Current unencumbered RN license for Ohio.
• Minimum 1 year acute care/medical-surgical nursing experience.
• Hospice experience preferred.
• Proficiency with Windows and MS Office; EMR exposure required.
• Must read, write, speak English fluently and clearly.
• Valid driver's license and insurability required.

Responsibilities

• Care Management and Service Delivery – 60%: assess patients, plan and implement care in homes and facilities, coordinate with providers.
• Quality and Compliance – 30%: maintain accurate records per guidelines and participate in quality projects.
• Other duties as assigned; ensure regulatory compliance and patient-focused care.
