Job summary

Gentiva in Indiana is seeking a dedicated Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients' homes or other living environments. You will assess, treat, educate, and support patients and families while coordinating with an interdisciplinary team to ensure compassionate, compliant care.

You will perform assessments, manage symptoms, update plans of care, and document care while maintaining infection control standards and regulatory requirements.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• CPR certification and valid driver’s license with auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams including physicians and other providers.
• Educate patients and caregivers on disease process, end-of-life care, medications, nutrition and self-care.
