Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, patient-centered home-based care across Indiana. You will assess, treat, and educate patients and families, coordinating with physicians, social workers, and chaplains to develop and refine personalized plans of care.

The role emphasizes symptom management, documentation, and timely communication with the care team, with on-call duties as needed. Indiana licensed RN with palliative care experience preferred.

Qualifications

• RN licensure in the state of employment.
• Hospice or palliative care experience preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing RN visits.
• Deliver skilled nursing care per Plan of Care in home or local facilities.
• Coordinate with IDG to update care plans and ensure timely interventions.
• Educate patients and families on disease processes, symptom management and end-of-life care.
• Maintain accurate clinical records and adhere to infection control standards.
• Participate in on-call and patient support activities as applicable.
