Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to provide compassionate, patient-centered end-of-life care in patients’ homes or preferred settings. You will assess needs, develop individualized plans, and coordinate with an interdisciplinary team to ensure high-quality care.

Responsibilities include delivering skilled nursing interventions, educating patients and families, and maintaining thorough documentation while adhering to regulatory guidelines and quality standards.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year of RN experience in acute care, hospice, home health, or oncology.
• Hospice or palliative care experience preferred.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Update and maintain individualized plans of care including visit frequency.
• Coordinate with physicians, LPNs, CNAs, social workers, chaplains, and others.
• Educate patients and caregivers on disease processes, medications, and end-of-life care.
