Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead in-home patient care, coordinating plans of care with the interdisciplinary team. You will educate families, document clinical notes, and monitor symptoms, ensuring safe, compassionate end-of-life care.

Responsibilities include assessments, medication guidance, and ongoing communication with physicians, social workers, and chaplains. A valid RN license and hospice experience are required; travel to patient homes is common.

Qualifications

• Registered Nurse with an active state license.
• Minimum 1 year experience in hospice, palliative care, or home health.
• Ability to perform comprehensive patient assessments and plan care.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care in patient homes or other locations.
• Perform initial and ongoing assessments, adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers and chaplains to refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Teach caregivers practical skills and offer reassurance.
• Communicate updates to the care team and document in medical records.
• Maintain infection control and compliance with regulatory standards.
• Support quality improvement initiatives and care scheduling to ensure consistency.
