Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead in-patient and home-based end-of-life care. You will assess patients, implement and adjust plans of care, and collaborate with an interdisciplinary team to provide compassionate, comprehensive support to patients and families.

Responsibilities include patient assessments, symptom management, caregiver education, and timely documentation. Experience in hospice or palliative care is preferred; CPR certification is required.

Qualifications

• RN license is required to practice in the state of employment.
• Minimum 1 year of RN experience in hospice, palliative care, or home health.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, IDG members, and other clinicians.
• Educate patients and caregivers on disease, self-care, medications, nutrition, and end-of-life care.
• Communicate changes in condition to the care team and ensure timely interventions.
• Maintain timely clinical documentation and comply with infection control standards.
• Support quality improvement and transitions in levels of care.
