Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to provide in-home nursing care, assessments, and care coordination for patients with life-limiting illnesses. You will educate families, implement plans of care, and collaborate with physicians, social workers, and the broader team to ensure comfort and dignity.

Qualifications

• RN license in the state of employment.
• CPR certification.
• Current automobile insurance and valid driver’s license.
• Minimum 1 year RN experience in hospice, home health, acute care or oncology.
• Hospice/palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educate patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency and risk ID.
• Coordinate care across interdisciplinary teams with physicians and other staff.
• Provide education on disease, self-care, end-of-life care, medications and nutrition.
• Communicate condition changes to the care team for timely interventions.
• Support continuity of care and transitions across care settings.
• Document timely clinical records including orders, notes and plans.
• Maintain relationships with IDG and involve team as needed for outcomes.
• Participate in on-call and patient support activities as applicable.
