Job summary

Gentiva is seeking a compassionate Hospice RN Case Manager to lead patient care in home settings. You will assess patients, administer skilled nursing interventions, and continually update care plans in collaboration with an interdisciplinary team of physicians, LPNs, CNAs, social workers, and chaplains.

Qualifications

• RN license in Texas required.
• Hospice or palliative care experience preferred.
• Strong patient education and care planning skills.
• Ability to collaborate with IDG and physicians.
• Excellent documentation and communication skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease processes, medications, and end‑of‑life care.
• Document timely clinical records and maintain infection control standards.
• Participate in on‑call and patient support activities as applicable.
