Job summary

Gentiva Hospice in Ohio is seeking an experienced Hospice RN / Case Manager to lead patient care in home or facility settings. You will act as the primary liaison for patients and families, delivering skilled nursing care aligned with each Plan of Care.

Collaborate with an interdisciplinary team, conduct assessments, educate families, and document care while maintaining compliance and infection control. This role offers meaningful work in end-of-life care with comprehensive benefits.

Qualifications

• Must be a Registered Nurse with current license to practice in the employing state.
• Minimum of one year RN experience in hospice, home health, or related settings.
• CPR certification required.
• Valid driver's license and auto insurance.

Responsibilities

• Provide skilled nursing care per Plan of Care in home or equivalent setting.
• Perform initial and ongoing patient assessments.
• Collaborate with IDG to develop personalized care plans.
• Educate patients and families on disease, treatment, end-of-life care.
• Maintain timely clinical documentation and infection control compliance.
• Participate in on-call shifts as applicable.
