Job summary

Gentiva, a hospice care provider, is seeking a Hospice RN / RN Case Manager to lead patient care in home settings. You will be the primary contact for patients and families, coordinating with an interdisciplinary team to deliver and adjust the Plan of Care.

You will perform comprehensive assessments, provide skilled nursing interventions, educate caregivers, document care, and support patients and loved ones through end-of-life processes with compassion and professionalism.

Qualifications

• Experience in hospice or palliative care preferred.
• Current RN license in the state of employment.
• CPR Certification must be maintained.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each Plan of Care in home or other settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide education, comfort, and bereavement guidance to patients and families.
• Educate caregivers on safe, confident home care practices.
