Job summary

Gentiva Hospice in Sidney, Ohio seeks a dedicated Registered Nurse (RN) to join our hospice team delivering compassionate, in-home care. The RN will lead assessments, implement and document plans of care, and collaborate with an interdisciplinary team to support patients and families at end of life.

Requirements include an active RN license, a minimum of one year of RN experience (hospice or home health preferred), and CPR certification.

Qualifications

• RN license in good standing.
• Hospice or home health experience preferred if possible.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver and document skilled, hands-on nursing care based on each patient’s Plan of Care—in home settings or wherever they call home.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build and refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families, with education and bereavement guidance.
