Job summary

Gentiva is seeking a Registered Nurse (RN) Hospice for a nursing role in Washington Court House, Ohio. The RN will lead personalized hospice care at patients' homes, collaborating with physicians, LPNs, CNAs, social workers, and chaplains to implement plans of care and ensure high-quality, compassionate end-of-life services.

The role requires an active RN license, CPR certification, and hospice or palliative care experience is preferred.

Qualifications

• Current license to practice nursing in the state of employment
• CPR Certification
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments
• Deliver skilled nursing interventions and symptom management aligned with goals of care
• Develop and update individualized plans of care including visit frequency
• Coordinate care across interdisciplinary teams including physicians and nurses
• Provide patient and caregiver education on disease, self-care, end-of-life care, medications and nutrition
