Job summary

Gentiva is seeking a compassionate Hospice RN / Case Manager to lead patient care in home settings across Johnson City, TN. You will assess patients, administer skilled nursing care, and develop personalized plans of care in collaboration with physicians, LPNs, CNAs, social workers, and chaplains.

You will educate patients and families, coordinate with the IDG, and ensure accurate documentation while upholding regulatory standards and infection control guidelines.

Qualifications

• RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Strong clinical assessment and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and reassessments, providing education to patients, family members and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease processes, end-of-life care, medications, nutrition and dietary needs.
• Communicate changes in condition to the care team and ensure timely interventions.
