Job summary

Gentiva Hospice seeks an experienced Registered Nurse to join our team as a full-time Hospice RN. You will perform in-home assessments, deliver skilled nursing care, and educate patients and families on end-of-life care, medications, and comfort measures.

Collaboration with an interdisciplinary team is essential to develop and implement individualized care plans. The role requires strong assessment skills, empathy, and compliance with regulatory standards.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year RN experience in hospice, home health, acute care, oncology, long-term care, or related fields.
• Hospice or palliative care experience preferred.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home, hospice, or related settings.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams with physicians and IDG members.
• Provide education to patients and caregivers on disease process, end-of-life care, medications, nutrition, and self-care.
• Communicate changes in condition to the care team and appropriate providers.
• Support transitions between levels of care and referrals as needed.
• Document timely clinical records including assessments, orders, progress notes, and care plans.
• Maintain strong relationships with the IDG and involve team as warranted.
• Participate in on-call and patient support activities as applicable.
