Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead skilled nursing care in patients' homes, assisted living, or other settings. You will perform assessments, coordinate with interdisciplinary teams, and guide families through the hospice journey with empathy and expertise.

The role emphasizes education, care planning, and timely documentation, while maintaining compliance with regulatory standards.

Qualifications

• RN license in the state of employment
• CPR Certification
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency based on patient needs.
• Coordinate care across interdisciplinary teams and consult with physicians and other providers.
• Educate patients and family members on disease process, self-care, end-of-life care, medications, nutrition and dietary needs.
• Document timely clinical records and ensure infection control and compliance standards.
