Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead individualized care at patients' homes and in supported living settings. You will conduct assessments, develop care plans, and coordinate with an interdisciplinary team to ensure compassionate, quality hospice services across life-limiting illness trajectories.

Current RN license in the state of employment, CPR, and a valid driver’s license are required; hospice experience is preferred.

Qualifications

• RN license in state of employment.
• Minimum 1 year RN experience; hospice/palliative care preferred.
• CPR certification required; valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facilities; provide patient and caregiver education.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, chaplains, and other team members.
• Document clinical findings timely and maintain accurate records; follow infection control standards.
• Support on-call duties and assist with transitions across care settings.
