Gentiva is seeking a licensed Registered Nurse to join our hospice and palliative care team as an RN Case Manager. You will conduct in-home assessments, provide skilled nursing care, and collaborate with an interdisciplinary team to implement personalized care plans.

You will educate patients and families on medications, symptom management, and end-of-life care, while coordinating with physicians and other caregivers to ensure compassionate, compliant care across settings.

Formación

• Must have RN license in the state of employment.
• 1 year of RN hospice or home health experience preferred.
• CPR certification and clean driving record required.

Responsabilidades

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease processes, medications, and end-of-life care.
