Gentiva Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager, delivering skilled nursing care in patients' homes and coordinating with an interdisciplinary team to advance goals of care. You will assess, educate, and support patients and families through the hospice journey.

The role emphasizes compassionate, patient-centered care, adherence to regulatory guidelines, and collaboration with physicians, social workers, and chaplains.

Formación

• RN license to practice in the state of employment.
• Minimum 1 year RN experience in acute care, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR certification required.
• Current automobile insurance and valid driver’s license.

Responsabilidades

• Serve as the primary contact for patients and families and guide them through hospice care.
• Deliver skilled nursing care at patients' homes or other locations.
• Perform initial and ongoing assessments and update plans of care.
• Collaborate with IDG and physicians to coordinate care.
• Document clinical records and ensure regulatory compliance.
• Provide education to patients and caregivers on disease, end-of-life care.
• Participate in on-call and patient support activities.
