Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate, in-home nursing care. You will assess patients, implement symptom management, and collaborate with physicians, LPNs, CNAs, social workers and chaplains to tailor individualized plans of care.

Family education and bereavement support are integral parts of the role. You must hold a current RN license and have at least 1 year of RN experience, with hospice or palliative care background preferred.

Qualifications

• Registered nurse with active state license.
• Minimum of one year RN experience in hospice, palliative care, acute care, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments in home or care settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequencies.
• Coordinate care with an interdisciplinary team and physician partners.
• Educate patients, families, and caregivers about disease, end-of-life care, medications and nutrition.
