Job summary

Gentiva is seeking a compassionate Hospice Registered Nurse / RN Case Manager to lead patients and families through the hospice journey at home or in other care settings. You will deliver hands-on nursing care, assess patient needs, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to craft individualized care plans while providing education and bereavement support.

The role emphasizes clinical excellence, empathy, and strong documentation to ensure quality end-of-life

Qualifications

• Active, current RN license in the state of employment.
• CPR certification.
• At least 1 year of RN experience in acute care, hospice, or home health.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Coordinate care with an interdisciplinary team to optimize patient outcomes.
• Educate patients and caregivers on disease process, medications, and self-care.
• Maintain accurate clinical documentation and ensure regulatory compliance.
