Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home and facility settings. You will assess needs, deliver hands-on nursing care, and guide families through the hospice journey with an emphasis on comfort and dignity.

Requirements include RN licensure and relevant experience; hospice or palliative care experience is preferred. You will collaborate with an interdisciplinary team to tailor plans of care and provide bereavement support where appropriate.

Qualifications

• RN and state licensure in good standing.
• Hospice or palliative care experience preferred.
• Ability to perform comprehensive patient assessments and education.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care across IDG and coordinate with physicians and other team members.
• Educate patients and caregivers on disease, self-care, medications, nutrition, and end-of-life care.
