Job summary

Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to deliver expert nursing care in patients’ homes or other living settings. You will perform assessments, implement plans of care, and coordinate with an interdisciplinary team while educating families and providing emotional support.

Current RN license and CPR certification required; Hospice or home health experience preferred. This is a full-time role with benefits, travel, and on-call responsibilities.

Qualifications

• RN with current license in the state of employment.
• Minimum 1 year RN experience in hospice/home health preferred.
• CPR certification.

Responsibilities

• Provide and document skilled nursing care per Plan of Care in home or other living setting.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with interdisciplinary team to refine care plans.
• Educate patients, families, and caregivers; provide bereavement support.
• Maintain clinical documentation and ensure infection control compliance.
• Coordinate with physicians, LPNs, CNAs, social workers, and chaplains.
• Communicate condition changes to care team and manage on-call duties.
