Job summary

Gentiva Hospice seeks a qualified RN Case Manager to lead in-home hospice care. The role involves comprehensive patient assessments, implementing plans of care, and educating families. You will collaborate with physicians, LPNs, CNAs, social workers and chaplains to ensure compassionate, compliant end-of-life care.

Qualifications

• Registered Nurse with active state license in the assignment state.
• Minimum 1 year RN experience in acute care, oncology, hospice, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk ID.
• Coordinate care with physicians and other IDG members.
• Educate patients and caregivers on disease, self-care, meds and end-of-life care.
• Communicate changes in condition to the care team for timely interventions.
• Document clinical records and maintain regulatory-compliant paperwork.
• Support continuity of care and transitions between care levels.
• Participate in on-call and patient support activities as applicable.
