Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient assessments and coordinate comprehensive, compassionate hospice care in patients' homes. You will work with an interdisciplinary team to implement personalized care plans and support families through challenging times.

Ideal candidates hold a valid AZ RN license with hospice or palliative care experience; strong assessment, documentation, and communication skills are essential.

Qualifications

• Current license to practice nursing in the state of requested employment (AZ).
• Hospice or palliative care experience preferred.
• Minimum of one year of RN experience in acute care, hospice, or home health.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care at home or other settings per the Plan of Care.
• Perform initial and ongoing RN assessments; monitor patient condition and adjust care as needed.
• Collaborate with the interdisciplinary team to build and refine personalized care plans.
• Provide education and support to patients and caregivers, including bereavement guidance.
