Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead in-home hospice and palliative care, coordinating a comprehensive Plan of Care. You will work with an interdisciplinary team to ensure timely, compassionate treatment and caregiver education.

Responsibilities include assessments, documentation, and ongoing support for patients and families, with emphasis on symptom management, education, and coordination across care settings.

Qualifications

• RN with state license and hospice/home health experience preferred
• Capable of delivering skilled nursing care in patients' homes or facilities
• Strong communication with patients, families and IDG

Responsibilities

• Be the primary contact for patients and families, guiding them through every step of the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care in the home or other settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers and chaplains to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families and educate caregivers.
• Communicate updates to the care team and ensure timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate, efficient care.
• Educate patients and families on medication administration, hospice services, and end-of-life processes.
