Job summary

Gentiva Hospice in Arizona seeks an experienced Registered Nurse Case Manager to lead in-home hospice care, perform assessments, and develop plans of care.

You will coordinate with physicians, LPNs, CNAs and social workers to deliver compassionate, patient-centered care, while documenting visits and ensuring regulatory compliance.

This position offers a flexible schedule with per diem options and a supportive team dedicated to comfort and quality end-of-life care.

Qualifications

• RN with state license
• Hospice or palliative care experience preferred
• Ability to perform comprehensive in-home assessments and education

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care with visit frequency
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains
• Educate patients, families and caregivers about disease, self-care and end-of-life care
• Document timely clinical records and ensure regulatory compliance
• Participate in on-call and patient support activities
