Job summary
Hospice of the West is seeking a Full Time Registered Nurse Case Manager (RNCM) to join our compassionate nursing team delivering end-of-life care across the community. You will provide hands-on hospice nursing, coordinate services, and guide patients and families through every step of the hospice journey.
As part of our interdisciplinary care team, you will collaborate with the Clinical Director to assess eligibility, advocate for patients, and ensure comfort and quality of life with clinical
Qualifications
- Current, unencumbered RN license.
- Minimum 1 year RN experience in med-surg/acute care; hospice a plus.
- Computer proficient with EMR experience.
- CPR certification if required by state.
- Valid driver's license and dependable transportation.
Responsibilities
- Respond to clinical referrals with compassionate, timely care to support excellent service.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Coordinate direct patient care to maintain comfort and quality of life.
- Deliver nursing care rooted in proven principles to ease pain and distress.
- Educate patients and families to participate in personalized care plans.
- Share on-call duties to be there when it matters most.
