Hospice of the West is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team serving Casa Grande and surrounding areas.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability for hospice services.
• Nurturing Direct Care: Deliver hands-on hospice nursing care and coordinate services to ensure patient comfort and quality of life.
• Educator and Advocate: Educate patients and families to create personalized care plans aligned with goals.
• Call of Compassion: Assist with on-call duties to provide care when it matters most.
