Job summary
Holisticare Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing end-of-life care throughout the community. This full-time role features a Monday–Friday schedule, 8:00 am–4:30 pm, serving the Monmouth area with a focus on coordinating services and guiding patients and families through the hospice journey.

Qualifications
- Active RN license is required.
- Minimum 1 year of RN experience in medical-surgical/acute care.
- Hospice experience is a plus.
- Must be computer proficient and able to use EMR systems.
- CPR certification may be required by state.
- Must have a valid driver’s license and reliable transportation.
Responsibilities
- Engage with clinical referrals with care and expertise to answer inquiries.
- Collaborate with Clinical Director to assess client eligibility for hospice services.
- Provide direct nursing care while coordinating services to maintain comfort and quality of life.
- Deliver nursing care rooted in evidence-based principles and compassionate bedside support.
- Educate patients and families to participate in personalized care planning.
- Share call duties to ensure availability when it matters most.
