Job summary

Hospice of the West is seeking a Full Time Hospice Registered Nurse Case Manager to join our nursing team delivering end-of-life care in Mesa and surrounding areas. The role emphasizes compassionate patient support, care coordination, and educational guidance for families, with an 8am-5pm weekday schedule and occasional on-call duties.

The RNCM will collaborate with the Clinical Director and interdisciplinary team to ensure comfort, dignity, and quality of life for patients while navigating

Qualifications

Must hold a current, unencumbered RN license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
Proficient with EMR and basic computer tools.
Valid driver's license and reliable transportation.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Deliver hands-on hospice nursing care and coordinate services.
Provide education to patients and families for personalized care plans.
Participate in on-call duties as part of the team.
