Job summary

Sanctuary Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem to support patients and families with end-of-life care across our community. You will coordinate care, address needs compassionately, and ensure comfort and quality of life for patients.

You will collaborate with the Clinical Director, provide direct nursing care, educate families, and participate in on-call duties within a supportive team dedicated to dignified, patient-centered hospice care.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in EMR systems and computer typing.
• CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to referral information with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct patient care to maintain comfort and quality of life.
• Deliver nursing care rooted in proven principles and techniques.
• Educate patients and families to participate in care planning.
• Share on-call duties with the team.
