Job summary

Hospice of the West in Mesa, AZ is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team delivering end‑of‑life care across the community.

The role features a Monday–Friday 8 am–5 pm schedule with occasional on‑call and requires an active RN license, at least 1 year of medical‑surgical or acute care experience, EMR proficiency, CPR, and reliable transportation.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
