Job summary

Hospice of the West is seeking a full-time Hospice RN Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care across the community. Monday–Friday, 8 am–5 pm, West Valley territory, with hands-on care and care coordination.

You will work with an interdisciplinary team, guiding patients and families through their hospice journey while maintaining clinical excellence, compassion, and connection. Competitive benefits and onboarding provided.

Qualifications

Hold a current RN license.
Minimum 1 year of med-surg/acute care nursing experience; hospice a plus.
Must be proficient with computers and EMR systems.
CPR certification may be required by state.
Current driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries with compassion.
Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
Nurse with direct care, focusing on patient comfort and quality of life.
Provide nursing care rooted in evidence-based practices to ease pain and promote comfort.
Educate patients and families, empowering them to participate in personalized care plans.
Share call duties as part of the team to ensure timely patient support.
