Job summary

Continuum Hospice & Palliative Care Massachusetts is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our caring nursing team. You will provide hands-on end-of-life care, coordinate services, and guide patients and families through their hospice journey in the South Boston area.

Qualifications

Active RN license in MA or eligibility.
Minimum 1 year RN experience in medical-surgical/acute care.
Hospice experience is a plus.
CPR certification if required by state.
Valid driver’s license with reliable transportation.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess client eligibility.
Provide hands-on hospice nursing and coordinate services.
Deliver nursing care focused on comfort and quality of life.
Educate patients and families to create personalized care plans.
Participate in on-call rotations and team communication.
