Job summary

Corewell Health Hospice seeks a Nursing Case Manager to coordinate end-of-life care and symptom management for patients with a limited life expectancy. You will assess needs, develop individualized plans, and collaborate with an interdisciplinary team to ensure high-quality hospice care.

Under Nursing - Hospice West, you will travel to homes, facilities, and hospitals, document care, educate families, and provide compassionate support while upholding hospice standards and safety practices.

Qualifications

• Associate’s degree in nursing.
• Michigan RN license upon hire.
• State of Michigan Driver’s license upon hire.
• 2 years of relevant experience.

Responsibilities

• Patient assessment and care planning based on goals of care.
• Symptom management and documentation of patient progress.
• Coordinate with physicians, social workers, chaplains and other team members.
• Educate patients and families on disease progression and hospice options.
• Provide emotional and psychosocial support to patients and families.
• Travel to patients’ homes, nursing facilities, and hospitals for direct care.
• Maintain compliant records and participate in safety initiatives.
