Job summary

Holisticare Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing end-of-life care in the community. Schedule is Monday–Friday, 8 am–4:30 pm, covering the Monmouth area.

You will deliver hands-on hospice nursing, coordinate services, and guide patients and families with compassion and clinical excellence. Qualified candidates hold an active RN license, at least 1 year in medical-surgical/acute care (hospice a plus), EMR

Qualifications

Hold current unencumbered license as a Registered Nurse.
Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
Must be computer proficient in typing and various programs, including background in EMR.
Possess and maintains current CPR certification if required by state.
Current driver's license, vehicle insurance, and access to dependable transportation.
