Job summary

Wellstar Health System is seeking a Hospice RN Case Manager to coordinate care for terminally ill patients and their families. You will assess, plan, and implement nursing care in collaboration with physicians and the hospice team, providing physical and psychosocial support to patients and caregivers.

The role requires strong clinical judgment, excellent communication, and the ability to navigate end-of-life care discussions while maintaining high standards of comfort and dignity for patients

Qualifications

• Associates or Diploma in Nursing required.
• Bachelor’s degree in Nursing preferred.
• RN license required (single state or compact).
• BLS certification required (or ARC-BLS).
• Georgia Driver's License (or valid state license).

Responsibilities

• Provide ongoing patient care per interdisciplinary care plan.
• Maintain contact with patients/families via calls and visits.
• Manage medications and educate on administration and safety.
• Assess symptoms and communicate with physician for adjustments.
• Document care and update plan of care for each patient.
• Prepare patient and family for death; support meaningful end-of-life planning.
• Collaborate with interdisciplinary team and support staff.
